It's a version of the tessera sanitaria that also works as a Carta Nazionale dei Servizi: a digital identity card with a chip. With a reader and a PIN, you can use it to log into public services like SPID does.
If you don't want SPID and your CIE isn't ready yet, the TS-CNS can be your digital identity tool. Many regions issue it by default.
The chip needs to be activated at your ASL and you need a reader (or a smartphone with NFC and the right app). Without activation, the card is just a normal tessera sanitaria.
